Malawi 2020 Spectrum file, version 5

Updated with latest ANC prevalence (up to end Q3 2019) and program data (extrapolated up to end 2019). Fixed several small issues with ANC RT prevalence data in 2019 Spectrum file (key stroke error for N in 2 sites, completed 2018 data with Q4 2019 data which was missing, excluded several site/year data points where HIV ascertainment was <90%). Re-fitted EPP with default options.
